Submit Your Claim

Submit your Claim Form and Consent to Sue securely online to receive a payment from the settlement. You will need your Notice information to get started. Claims must be submitted within 60 days of the date your notice was issued.

Prefer to submit by mail or e-mail? You may also return your signed Claim Form and Consent to Sue by mail to the Settlement Administrator at P.O. Box 2031, Tustin, CA 92781, or by e-mailing it to info@ilymgroup.com. If you have questions about your ILYM ID or your eligibility, call the Settlement Administrator at (888) 250-6810. All claims must be submitted no later than 60 days after the date your notice was issued. If you do nothing, you will not be covered by the settlement and will not receive a payment.
